After downloading all components and running the EPM 11.1.2.1 installation tool, the only option available is for a new installation. The Apply Maintenance Release is grayed out and not available.

Cause

The reason the Mainentance option was unavailable was because the installation program could not locate the .oracle.instance file. The 11.1.2.1 installation program looks for that file in the users home directory during the installation process. This file did not exist in the users home directory.

Failure to have this file in the home directory will prevent application of the maintenance release.

In this particular case the previous 11.1.2.0 installation was performed as the same user, but they had right-clicked on the install tool and selected to Run As Administrator. So in effect it was run as he user Administrator and not the user they thought. This action would cause the .oracle.instance file to be put in the Administrators home directory and not the users HOME directory.
Solution

There are two solutions to this issue:
A - Re-run the install tool as the user who performed the initial installation
B - Copy the .oracle.instance file from other users directory to the HOME directory of the user doing the new installation.
